About this worksheet
A permission slip is a form, and a form is read differently from a story: each line pairs a label with the fact that answers it, and the bottom asks for a signature. This worksheet teaches students to read a whole form, including the due date, before anything is signed.
The reference box, the permission slip, and Section A are all on page 1. The reference box explains how a form pairs labels with facts and why the due date matters. Section A has eight questions answered directly from the document. Section B on page 2 has four short-response questions.
Page 3 is a complete answer key with answers for every item.
